1. Acceptance of These Terms

These Terms of Service (the “Terms”) govern your access to and use of the Undeez Urologist Directory, available at undeez.co/directory (the “Directory”), which is operated by Care For There LLC (“Care For There,” “we,” “us,” or “our”). By accessing or using the Directory, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Directory.

2. The Directory Is Informational Only — Not Medical Advice

The Directory is provided for general informational purposes only. It is a listing of urologists in the United States and does not provide medical advice. Nothing in the Directory is a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions you have regarding a medical condition. Never disregard professional medical advice, or delay seeking it, because of something you have read in the Directory.

If you think you may have a medical emergency, call your doctor or 911 immediately. The Directory does not recommend or endorse any specific physician, and a listing is not a referral.

3. Directory Content and Accuracy

Physician listings are compiled from public sources, primarily the NPPES NPI Registry and Google. Profile descriptions and summaries are generated automatically and may be incomplete, out of date, or inaccurate. Ratings shown on the Directory are cited from Google and are attributed as such; they are not ratings created or verified by Care For There, and their display does not constitute an endorsement.

We do not warrant that any information in the Directory is accurate, complete, or current. If you are a physician and the information shown for you is incorrect, you may request a correction or removal by emailing support@undeez.co.

4. No Doctor-Patient Relationship

Use of the Directory does not create a doctor-patient relationship between you and Care For There or between you and any physician listed in the Directory. Communications made through the Directory are not confidential medical communications.

5. Eligibility

You must be at least 18 years old to use the Directory. By using the Directory, you represent that you meet this requirement and that you have the legal capacity to enter into these Terms.

6. Physician Profile Claims and Accounts

Physicians may claim their profile to manage the information shown and to access the doctor portal. Claiming a profile requires verification, which may include matching a National Provider Identifier (NPI). By claiming a profile or creating an account, you represent and warrant that you are the physician identified in the profile or an individual authorized to act on that physician's behalf, and that the information you provide is accurate.

You are responsible for maintaining the security of your account and for all activity that occurs under it. You agree to notify us promptly of any unauthorized use. We may suspend or refuse a claim that we believe is inaccurate, fraudulent, or unauthorized.

7. Subscriptions, Billing, and Cancellation

Claiming a profile is free. We also offer paid subscription plans that unlock additional features. The plans, their features, and their current prices are shown on the Directory at the point of purchase. Prices are stated in U.S. dollars.

Paid plans are recurring subscriptions billed in advance through our payment provider, Stripe, on a monthly basis. By subscribing, you authorize us and Stripe to charge your payment method on each renewal date until you cancel. Subscriptions renew automatically unless cancelled before the renewal date.

You may cancel a subscription at any time. Cancellation stops future renewals; it takes effect at the end of the current billing period. Except where required by applicable law, payments already made are non-refundable, and we do not provide refunds or credits for partial billing periods. We may change plan features or pricing on a going-forward basis; if we change the price of an active subscription, we will give you advance notice and the change will apply at the next renewal.

8. Reviews and User Content

The Directory may allow visitors to submit reviews and allow physicians to respond to them (together, “User Content”). If you submit User Content, you represent that it reflects a genuine experience, that it is accurate, and that it does not violate these Terms or any law. You must not submit content that is false, misleading, defamatory, harassing, obscene, infringing, or otherwise unlawful, and you must not submit a review in exchange for compensation or submit a review of yourself or a competitor.

You retain ownership of your User Content. By submitting it, you grant Care For There a non-exclusive, worldwide, royalty-free, sublicensable license to host, display, reproduce, and distribute that content in connection with operating and promoting the Directory. We may, but are not obligated to, review, moderate, edit, or remove any User Content at our discretion, including content we believe violates these Terms.

9. Acceptable Use

You agree not to:

• scrape, harvest, or use automated means to extract data from the Directory in bulk;
• interfere with, disrupt, or attempt to gain unauthorized access to the Directory or its systems;
• impersonate any person or misrepresent your affiliation with any person or entity;
• submit a profile claim that you are not authorized to make;
• use the Directory to send spam or to violate the privacy or rights of others; or
• use the Directory for any unlawful purpose or in violation of these Terms.

10. Intellectual Property

The Directory, including its design, text, graphics, and software, and all related intellectual property, is owned by Care For There or its licensors and is protected by law. Except for User Content, which is governed by the license above, you may not copy, modify, distribute, or create derivative works from the Directory without our prior written permission.

11. Third-Party Services and Links

The Directory relies on third-party services, including Stripe for payment processing and Google for analytics and place data, and may link to third-party websites such as individual practice websites and the Undeez store. We do not control these services and websites and are not responsible for their content, products, or practices. Your use of a third-party service is governed by that party's own terms.

12. Disclaimers

The Directory and all content in it are provided on an “as is” and “as available” basis, without warranties of any kind, whether express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Directory will be uninterrupted, secure, or error-free, or that any information in it is accurate, complete, or current.

13. Limitation of Liability

To the maximum extent permitted by law, Care For There and its officers, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of profits, data, or goodwill, arising out of or related to your use of the Directory. To the maximum extent permitted by law, the total liability of Care For There for any claim arising out of or related to the Directory will not exceed the greater of the amount you paid to us in the twelve months before the claim or one hundred U.S. dollars (USD 100).

14. Indemnification

You agree to indemnify and hold harmless Care For There and its officers, employees, and agents from any claims, losses, liabilities, and expenses, including reasonable attorneys' fees, arising out of your use of the Directory, your User Content, or your violation of these Terms or of any law or the rights of a third party.

15. Termination

We may suspend or terminate your access to the Directory or to any account at any time, with or without notice, if we believe you have violated these Terms or to protect the Directory or its users. You may stop using the Directory at any time. Provisions that by their nature should survive termination will survive, including Sections 8 through 16.

16. Governing Law and Disputes

These Terms are governed by the laws of the State of California, without regard to its conflict-of-laws rules. You agree that any dispute arising out of or relating to these Terms or the Directory will be brought exclusively in the state or federal courts located in California, and you consent to the personal jurisdiction of those courts, except where applicable law provides otherwise.

17. Changes to These Terms

We may update these Terms from time to time. When we do, we will revise the “Last updated” date at the top of this page. If we make material changes, we will provide a more prominent notice where appropriate. Your continued use of the Directory after an update means you accept the revised Terms.
